Action URL

I'm trying to initiate a screen pop in my CRM based on the Incoming Calls Action URL. I can't seem to find a parameter to pass the incoming phone number in my URL, just the name. Is there a parameter I am missing, or is there another way I should be doing this?

Any help would be greatly appreciated!
